Match Reports are used for the following low level issues:

• Pitch Issues - markings, goals, etc
• Club Uniforms - issues with clashes of socks, shorts, shirts, etc.
• Lack of Changerooms
• Delayed Kicks Off

This form should not be used for:

• Send Offs
• Match abandonments
• Violent Conducts
• Referee and Assistant Referee Abuse
