Transfer the information from your draft copy of the Incident Report Form in to the fields below. Members are reminded that a separate report is required for each player. If the incident involves club officials and/or spectators lodge a single report describing the nature of involvement of all the non-playing individuals.
Members should refer to the Member Handbook which provides guidelines for reporting an incident. If the incident involves R1, R2, R3, R6 and R6M*, please contact the Vice-President immediately for further advice BEFOREyou submit your final report. Use the 'Save for Later' button at the bottom of the form before contacting the Vice President for advice. The save for later link will be sent by email to form respondents as well as display a link to copy and keep safe.

Saturday Games - Monday by 10am
Sunday Matches - Tuesday by 10am
Note: The following Send Off Codes do not require a Incident Report.

Is the incident an abandoned match or an offence against a match Official? If so, these incidents are serious. DO NOT immediately submit your report. Write a draft of your report and seek advice BEFORE you submit your final report. Contact the President, Vice President, Secretary, Appointments Secretary or Treasurer for advice. Incident Reports should be lodged within 48 hours of the match. The SUBMIT button on this page is configured to send your report to the Secretary of SAFRA and SAASL.
